You've completed the Survey, renamed and saved it, and you're ready for Submission.

Your downloaded Survey file includes your Survey and Petition Letter. That file and any supporting documents you submit with it are called your "Survey Packet".

You selected the submission choice for your Survey Packet at Question #39, where you decided whether you would be submitting by email, postal service or some combination of the two.

If you are submitting your Survey, or Survey and supporting documents by email and are familiar with attachments, please click on the email link below, attach the documents in your "NGE&P Survey" folder, and send.

If you are submitting your Survey, or Survey and supporting documents by email and are unfamiliar with attachments, here is how to attach a document or documents to an email on a PC:

Click on the link below. An email will appear with the addresses for ATSDR and DCS already inserted and your unique Household ID already in the "Subject" field. Click on the paper clip icon at the top of the email. Another window will appear. Click on "Desktop" and find the NGE&P folder. Click on the folder and then click the "Open" button. Now you can see the file(s) inside the folder. Double-click on a file to attach it to the email. The name of the file you selected will be visible in the field of the email titled, "Attach". If you want to attach more files from the folder to the same email, repeat this process as many times as needed. If you find your files are too big (take too long) to send all together in one email, you can send as many emails with attachments as you need. Just be sure to always use the link below because it automatically contains ATSDR and DCS's addresses and your unique Household ID in the "Subject" field.

Click here if you're submitting your Survey Packet and supporting documents (if any) to both ATSDR and DCS by email (and you're not using webmail).
