Purpose

This site is intended to assist entities regulated by the DHS - Division of Quality Assurance with identifying incidents, allegations, or suspected occurrences of misconduct that must be reported to the Department under s. 146.40, Wis. Stats., and ch. DHS 13, Wis. Admin. Code (state law).

This site is not intended to assist entities with identifying reporting requirements under federal law, or other sources of state law.
