Thank you for taking the time to complete this survey designed to enable Community Business to report on the impact of your This is Me in India programme and/or wider mental health activities in India in the last 12-18 months.

There are 2 parts to this survey:
Part 1: ASSESSING IMPACT (5 questions looking at 5 impact measures)

Simple Yes/No response

Part 2: TRACKING PROGRESS (14 questions looking at what you have done)

For each question we ask you to:

Indicate to what extent you have done this? (Yes, In Progress, No)
Provide any detail around what you have done
Highlight whether you are would like advice or support

Please provide as much detail as possible as this will contribute to the richness of the insights in our impact report.
