CACFP Meal Distribution Plan Update Survey

Completion of the Meal Distribution Plan (MDP) Update Survey is required  when changes occur in your meal distribution or if you plan to use any of the of the Nationwide Waivers not previously selected on your original MDP.


NOTE: Please reference the CACFP Meal Distribution Plan Checklist prior to beginning the survey. If you exit the survey before it is submitted, your progress will be lost.

If you need to submit your MDP as part of your renewal and have been directed to this site in error, please take the Meal Distribution Plan Survey.

 

The MDP Survey Update communicates to OSPI the changes you are making.
When completing this survey:
Select "Yes" to waivers you are intend to use but did not include on your original Meal Distribution Plan. 
Select "Yes, intent to use this waiver was submitted with my original MDP" for waivers you are using and  included on your original Meal Distribution Plan.
Select "No" For waivers you do not intend to use or are no longer using.
