Public Comment Form

Instructions:  Public Comments may be submitted by completing this ONLINE Submission form OR you may download a form here and then fax or email the form to submit your comments. Public Comment forms, whether submitted online or by fax or email, must be received in the City Clerk’s Office no later than 2 PM the day of the meeting.

You must provide your name and address on the form along with your comments. Your comments will be read into the record when the item specified is heard. Refer to the agenda posted online to enter the specific agenda item number(s) on the comment form.

IMPORTANT NOTE:  Public Comment forms, whether submitted online or by fax or email, must be received in the City Clerk’s Office no later than 2 PM the day of the meeting.  Comments submitted after 2 pm will not be accepted.   Instead, comments can be provided via telephone. See the meeting agenda for appropriate dial in numbers. Agendas are available through the City's Public Access Portal.
