Rules of Golf Exam - Testing Accommodation Request

Testing Accommodations Request Form

USGA is committed to providing accommodations to individuals with disabilities in accordance with the Americans with Disabilities Act who wish to take the optional exams offered as part of the USGA Rules of Golf workshops. Reasonable and appropriate testing accommodations will be provided to those individuals who timely submit a request for accommodations and provide documentation of a disability or medical condition demonstrating a need for the requested accommodations. 

Please fill out the below Testing Accommodation Request Form and provide the required documentation at least 14 days prior to the first day of your registered Rules of Golf workshop. Incomplete or late submissions may result in sufficient time for USGA to evaluate the request and provide the appropriate accommodations.   

Testing Accommodations Request Form must include the following:
  • signature and date by the individual requesting accommodations; 
  • indicate the disability or medical condition and the relevant functional limitation; and 
  • state the needed accommodations. *Note- requested accommodations should align with the identified functional limitation.
Supporting Documentation:

a.   Documentation of the disability/medical condition must be in the form of an evaluation report from an appropriate health care professional, and include the following:
  • current report from within the past five (5) years; 
  • official letterhead of the health care professional and the professional’s credentials, signature, address, and telephone number; 
  • the individual's name and date of evaluation;  
  • Information concerning the specific diagnostic procedures or tests administered. *Note, diagnostic methods used should be appropriate to the disability and in alignment with current professional protocol;
  • the results of the diagnostic procedures and tests, and a comprehensive interpretation of the results; 
  • the specific diagnosis of the disability, with an accompanying description of the individual’s limitations due to the disability; and
  • a summary of the complete evaluation with recommendations for the specific accommodations and how they will reduce the impact of the identified functional limitation.
b.    Documentation of any previous accommodations provided by educational institutions or other testing entities/agencies. 

All submission are treated as confidential and reviewed by the USGA, and may be shared by the USGA with medical professionals, solely for the purposes of evaluating your testing accommodation request.  Accommodations will not be noted on test results or shared with any unauthorized third party.
