To be completed by school or child care facility administrator
The Indiana Finance Authority (“IFA”) developed the Lead Sampling Program to help schools and child care facilities ("facilities") assess if there is a presence of lead in drinking water within their building. The IFA is now accepting applications for the program. Please complete an application for each facility.If you are applying on behalf of multiple facilities (i.e. a school district or child care franchise), you may submit a multiple facility application spreadsheet instead by requesting one from the IFA contact listed at the bottom of this page. Each facility application will be ranked based on the information that is provided in the application. Priority will be given to facilities that:
- Predominantly serve children six and younger
- Serve low to moderate income children
- Operate in a building that was built prior to 1988
- Can demonstrate an established and sustainable school or child care program with factors indicating that the building will continue to serve as a school or child care facility in the future
- Have not already participated in an IFA Lead Sampling Program
- Apply early in the application process
Any missing information is assumed to not meet the above priorities. Therefore please provide all requested information. An asterisk denotes required information.
