Arrow Stage Lines Ethic's Survey

We have created a resource that any team member can use; to report what they feel is a serious violation of our Company Culture Agreement, believes to be a breach of ethics, or has the potential to cause harm. 

If the concern poses an immediate threat of harm, the team member should call their supervisor, manager or Human Resources right away.

We encourage team members to report all concerns to their immediate supervisor. If that is not possible, then concerns should be reported to their manager, Regional Director, or Human Resources.   When none of these are possible, or the team member feels they cannot talk to any of the above mentioned, they may use this Ethics Survey process.

This form should only be completed when there are concerns of any miscommunications or actions that cause you to feel they are in violation of our company policies or causes you to operate a company vehicle in an unsafe manner. This is not be used for emergency purposes or other events that fall outside of the scope of this survey. Please work with your immediate supervisor to help resolve these isolated issues. All reports will be submitted as anonymous reports unless you enter your contact information. In the event you want to be contacted you will receive notification within 3 business days of your submission.  All anonymous complaints will be reviewed by the Company’s Ethics Committee and followed up in areas where the concern is directed. 

To ensure a timely response to your concern, please include all relevant details such as what happened, when it happened, where it happened, who was involved, who was present, and what everyone did and/or said.

We value our team members and will handle all complaints, disputes or other actions, in a courteous, fair, and timely manner.
