Patient Experience with Primary Care Services and Health Benefits

Please help Massachusetts Health Quality Partners (MHQP) better understand patients’ experiences of primary care services and healthcare benefits by filling out this survey. The answers you provide can help identify areas to improve patient experiences of healthcare. The survey is voluntary and takes about 10-15 minutes to complete. Your answers are private; we will not attach any identifying information with data that we share. However, we will group answers from all respondents and share them with Blue Cross Blue Shield of Massachusetts and Tufts Health Plan.

1. Did you have a primary care visit in the last 12 months? A primary care visit is a visit with a health care provider- your primary care provider- who takes care of your day-to-day healthcare needs.
