Lead in School Drinking Water: Kit Ordering and Registration Survey- original

Thank you for taking this survey and submitting your information to the Health Department. This survey will collect important contact information and help us determine the number of drinking water test kits to send your school. Please read all of the items below and check the boxes to indicate that you have read and understand the information. *This question is required.

Before beginning this survey, you will need school administrator contact information and your completed tap inventory available for reference.
Multiple schools cannot be entered into the same survey response. One survey must be completed for each school.
Details and step-by-step instructions for how to complete your tap inventory, collect water samples and return your samples to the laboratory are posted online at www.healthvermont.gov/leadtest-schools.
Schools are responsible for collecting water samples from taps that are used by other programs located within their building as well (for example, licensed child care providers, including pre-kindergarten, preschool and after school programs).
