14. Robyn suffered a concussion playing field hockey and has persistent headaches but otherwise
is doing well and is back in school. Can she be cleared to go back to play? *This question is required.

15. Bobby suffered his 3^rd concussion playing football this past weekend. His symptoms resolved
after 5 days. State championships are this upcoming weekend which is 3 days from now. he be cleared to play.
