We take your privacy seriously. Information collected during this survey process is confidential and will be used for the sole purpose of the feasibility study conducted by Meitler. Your personal information will not be given to the diocese or any third party. All responses will be coded with a response id. In addition, responses will be tagged with the user’s IP address and metadata to ensure minimal overlap of households. Where multiple corresponding entries arise, responses will be cross analyzed for a complete response for final qualification. Finally, any personal data collected (IP address, metadata, email address, name) will be separated from the database for the final analysis to assure anonymity. • You may save your survey entry, to complete at a later time, by clicking the button at the top of your screen and providing your email address. You will receive an individualized link sent to your email that will allow you to edit and continue your survey until completion. Your email will only be used for this purpose and will be removed from our final database prior to analysis.
• You may exit the survey at any time.
• Your response does not obligate you to enroll your children.
