This survey is being conducted by the Optical Research division of Jobson Medical Information LLC, 395 Hudson St, New York, New York 10014 (“we”). Anyone who completes the survey while the survey link is live is eligible to win the gift card. When we have enough completed surveys for the needed sample size, we will deactivate the link and conduct a random drawing of the names from all completed surveys to select one winner. We will notify the winner by e-mail and ask that person to reply by phone or e-mail and provide a street address. We will send the gift card to the address provided.
If the gift card is returned to us as undeliverable, it will not be awarded. The chances of winning depend on the total number of people who complete the survey by the time we deactivate the link. The winner is responsible for paying all federal, state and local taxes. All survey participants release us and our directors, employees, officers, parents, subsidiaries, and affiliates from any liability arising from their acceptance and/or use of the gift card.
