LeadingAge is continuing to
advocate for opportunities for our post-acute and long-term supports providers to play a leading role in new and evolving payment and service delivery reform and integrated service initiatives. This is more urgent now in the wake of the Center for Medicaid & Medicare Services’ (CMS) recent announcement to eliminate post-acute provider-initiated, bundled payment models.
To advance our integrated service vision and secure a lead role for our members in these efforts, it is vital that we establish a national database of our members’ activities to demonstrate to CMS and Congress the LeadingAge members' commitment to value-based care and other integrated service delivery system innovations by showing them what our members are doing.
This survey will collect data on:
1) Payment and service delivery innovation activities in which members are engaged.
2) The motivation for their engagement in activities.
3) Their experience so far.
Even if you do not yet participate in these activities, we encourage you to take this 15-20 minute survey to help us better understand where innovation is occurring and why, and what barriers still exist for members to participate in existing innovations or pursue integrated service delivery. What you get: Everyone who completes this survey will receive a “benchmark” report comparing their responses to the aggregated total response.
