Khaas Shiksha | Nomination Form

Instructions:

1. Please note that at least 2 and at most 5 nominations should be made by each block.

2. A teacher can only be nominated for a work done for Classes 1 to 8.

3. The activity nominated should not be more than 12 months old.

4. The document that you upload should act as a proof for the best practice that the teacher has performed. For eg. (i) If you are saying that the teacher uses TLM in class, upload a few photos of her performing the TLM in class. (ii) If you are saying that the teacher increased enrollment, upload an official document that acts as an enrollment proof

5. The description that you write should briefly describe exactly what the teacher has done differently and how she has done it.
For eg. A teacher nominated for increase in enrollment explained how the teachers used to go to the community everyday after 4 PM to identify community issues and guiding them. After identifying the problems as bad perception of the school, the medium was changed to English, ICT teaching was started in school and pamphlets were printed and distributed about the school facilities.

6. The best nomination would be chosen by the state and the best practices would:
o Feature on DEE website
o Shared with other teachers, block, district and state officials
o Receive Letter of Appreciation from the Director
o Be invited for presentation at District/State Level Meetings
o Feature in the Newspaper

7. The responsibility of the authenticity of the nominations rests with the BRCC(s)/BEEO(s)/DPO(s). In case a nomination is found to be fake, the state will hold the BRCC/BEEO/DPO responsible.

निर्देश:

1. गौर करे कि एक खंड द्वारा कम से कम २ और अधिकतम ५ अध्यापकों का नामांकन किया जा सकता है |

2. कक्षा १ से ८ में ख़ास शिक्षा-संबंधी कार्य करने वाले अध्यापक को ही नामांकित किया जा सकता हैं |

3. शिक्षक के ख़ास कार्य बीते 12 महीनों से पहले के नहीं होने चाहिए यानि कि गत 12 महीनों में ही संपन्न किये गये कार्य के लिये शिक्षक को नामांकित करें ।

4. अपलोड किये जाने वाले दस्तावेज़ अध्यापक के ख़ास कार्य को प्रमाणित करने का काम करेंगे |
उदाहरण (i) अगर आप कह रहे हैं की शिक्षक कक्षा में टी.ल.म. का प्रयोग करते हैं, तो उनकी टी.ल.म. के साथ पढ़ाते हुए कुछ फोटो अपलोड करें । (ii) अगर आप कह रहे हैं की शिक्षक ने स्कूल का नामांकन बढ़ाया हैं, तो नामांकन बढ़ने के सत्यापित पत्र अपलोड करें

5. 'अध्यापक के काम का विवरण' में अध्यापक ने क्या कार्य अलग किया और कैसे किया या कैसी कार्यशैली अपनाय, इसको संक्षिप्त रूप में बताए -
उदाहरण : नामांकन बढ़ाने के लिए यह लिख सकते हैं कि अध्यापक रोज़ शाम ४ बजे के बाद, समाज में जाकर, भिन्न भिन्न लोगों से बातचीत कर,उनके स्कूल के बारे में विचार समझें | उनके विचारों पर मंथन करने के बाद स्कूल का माध्यम अंग्रेजी कर दिया गया, ICT द्वारा पढ़ाई शुरू कर दी गयी, और स्कूल में उपलब्ध सुविधाओं के बारे में विज्ञानपन निकलवाए गए | इन वजहों से स्कूल का नामांकन २०% बढ़ गया |

6. राज्य समिति के द्वारा श्रेष्ठ गतिविधियाँ / कार्य चुने जायेंगे और उन्हे:
-> प्राथमिक शिक्षा विद्यालय की वेबसाइट पर दर्शाया जायेगा
-> अन्य शिक्षकों, ब्लॉक, जिला और राज्य अधिकारियों को बताया जायेगा
-> शिक्षा विभाग से प्रशंसा पत्र दिया जाएगा
-> जिला / राज्य स्तर की बैठकों में प्रस्तुतु करने शिक्षक को आमंत्रित किया जायेगा
-> समाचार पत्र में छापा जायेगा

7. नामांकन की प्रामाणिकता का उत्तरदायित्व बीआरसीसी / बीईईओ / डीपीओ  का है | यदि कोई नामांकन नकली पाया जाता है, तो संबंधित बीआरसीसी / बीईईओ / डीपीओ को जिम्मेदार ठहराया जायेगा |

8. चयनित नामांकनों का ब्योरा प्रारम्भिक शिक्षा विभाग की वेबसाइट पर उपलब्ध है | इन्हें देखने के लिए यहां क्लिक करें |
