Lewisham Youth Theatre's BIG Evaluation

Lewisham Youth Theatre (LYT) would like to know more about the impact that our work has on those who take part. We’ve developed this survey to help us measure the long-term impact of our work.

This survey is for anyone who has ever taken part in LYT’s projects. Whether you have taken part in one session or several projects we want to hear from you.

The survey should take you about 15 minutes. Once you submit it, you will be entered in a prize draw to win one of four £25 high street gift cards. Please make sure to give us your name and email at the end of the survey if you would like to be entered in this prize draw (winners will be announced in October 2016).

Please make sure to fill in the survey for the individual who has taken part with LYT. If more than one person in your household has taken part with LYT, please fill in a new survey for each participant.
