This survey is for all Mt. Level Missionary Baptist Church members and visitors ages 12 and over. As we continue to seek the Lord on how we are growing, stretching and changing as a congregation, we appreciate you taking the time to provide your honest and thoughtful responses.  This survey will take approximately 3 minutes to complete.
