The Roots to Health Survey is a short assessment of needs for good health. The survey includes questions about your work, housing, food, transportation, neighborhood, and social support, your level of stress, and your health behaviors, function, and capacity. This survey is designed to help health care systems understand when unmet basic needs may be negatively impacting health and health care quality.
