Your application was successfully submitted. The free pass will be available on the card you indicated on this form no later than 3 weeks from today. You can check your card status by tagging a Clipper® reader.

Please remind your child to hold the card to a Clipper reader each time they board a Muni vehicle or enter a Muni station faregate. Tagging the card reader serves as proof of payment. The card must be presented to a fare inspector or station agent upon request. Use of the card by anyone other than the individual it is registered to may result in a fine and confiscation of the card.

This pass will remain active for the duration of the program, or until the date of the pass holder’s 19th birthday, whichever comes first. The pass is not valid for special event service. Riding transit in San Francisco is a privilege. Please encourage your child to be respectful and courteous to others when they ride. Contact the Clipper Customer Service Center Bureau (1.877.878.8883) if your child’s Clipper card has been lost, stolen, or is not working. A $5 fee will be charged for a replacement card. Please note the pass will not load on a card that has not been registered to a youth between the ages of 5 and 19. If you provided an adult card number by mistake you will be notified by SFMTA via mail.

Pass may only be used by the individual to whom it is issued. Use of the pass is subject to the Clipper Privacy Policy (clippercard.com/Clipperweb/privacy.do). Transferring, selling or other improper use will result in confiscation of card, disqualification from program participation and/or possible fines.
