Participants from member URA institutions can apply for travel funds to cover partial costs for their trip.

1. Required questions are marked with an asterisk. if you leave a required question unanswered, you will get an error when you hit "Submit" button or 'Save and Continue" later.
2. The questionnaire can be filled out in multiple sittings. In order to do this, click on "Save and continue later" at the bottom of the page. You will be asked for
an email to send a unique link to be used to resume filling out the survey. PLEASE double check the provided email, to ensure successful delivery of the link and the a copy of your responses upon submission.
3. WARNING !!! You can click on "Submit" ONLY ONCE. Please do not click on "Submit" button if you wish to complete your survey later.
4. Upon submission a pdf file will be emailed by "SurveyGizmo (notifications@sgizmo.com)".
