Thank you for choosing to apply to become a Forsa mentor. The process begins with this application form, following which we will review and categorise applicants and then arrange a brief interview, either in person or by phone. This application form consists of 6 short pages of questions about you and your motivation to join the programme. It should take no more than 15-20 minutes to complete and you may save your progress and return later if you wish.
